United Airlines flight UA1394, a Boeing 737-724, lined up on the left side of the runway 10L during an intersection departure at Chicago-O'Hare, IL (ORD). The crew aborted the takeoff. They vacated the runway, taxied to the threshold of runway 10L and departed eight minutes later.

The pilot’s failure to maintain adequate airspeed after takeoff due to his distraction by the opening of the cabin door, led to the crash of a Beech A36TV Turbine Bonanza near Lancaster Airport (LNS), Pennsylvania, USA, in March 2025. This was the main conclusion from a newly released NTSB report.
The pilot and one passenger were seriously injured, and the other three passengers sustained minor injuries.
A factor in the accident was the airplane’s overweight condition, which increased its stall speed.

ADS-B data indicates that the aircraft was approaching Steamboat Springs Airport, CO (SBS). The last datapoint was 00:19 LT on the summit of 8252 ft high Emerald Mountain, 3 nm south of the runway 32 threshold.

TAP Air Portugal flight TP1240, an Airbus A320-251N, descended below the Minimum Sector Altitude during descent to the destination Prague (PRG).
The flight turned onto the extended centreline of runway 06 at 21 nm from the threshold.
It should have continued to the Final Approach Fix (at about 9 nm from the threshold) at an altitude of 4000 feet, but continued descending below that altitude and below the applicable Minimum Sector Altitude (MSA) of 3600 feet.
At that time the aircraft was descending over an area with 500 m (1640 ft) high hills.
Reportedly, the flight reached 1000 feet AGL before it climbed back to 4000 feet. A 360 degree orbit was flown before the flight repositioned on the approach course.
A safe landing was made at 10:26 UTC.

An Indonesia Air ATR 42-500 with 10 occupants on board, operated for the Marine and Fisheries Resources Surveillance (PSDKP), impacted a mountainside while approaching Makassar-Sultan Hasanuddin International Airport (UPG).
ATC gave vectors to the crew after noticing that the aircraft was not on the correct approach path.
The wreckage coordinates reported by the search and rescue agency Basarnas point to a 4327 feet high peak, 14 nm north-east of the threshold of runway 21 at Makassar. Minimum Sector Altitude in the area is 8500 feet.
