Requirements:
Motorcycle* (preferably owned)
At least 18 years old and above
Professional Driver's License
Valid NBI Clearance & Drug Test(At least 6 months)
OR/CR (under rider's name)
*if the motorcycle is not owned, please provide the following:
Authorization Letter (from the owner of the motorcycle)
2 Gov't issued ID's Photocopy (owner of the motorcycle)
Photocopy of OR/CR or Deed of Sale
