NOTICE TO THE PUBLIC

Please be informed that on 28 November 2016, the Regional Trial Court has approved the release of the Liquidation Values for claims (1st tranche distribution) of Permanent Plans, Inc. Planholders.

Listed below are names of Planholders who are entitled to the said distribution but not yet filed their documents.

In order to process your claim please submit the following requirements: Original Policy Contract; Original Certificate of Full Payment (if fully paid); photocopy of two (2) Gov't issued IDs with three (3) specimen signature. Permanent Plans, Inc. is currently located at Ground Floor Unit 4-A Dolmar Building, United Nations Ave., Paco, Manila. Notice is hereby given that Permanent Plans, Inc. (PPI) is still in process of distributing the liquidation value of plans to its planholders for the 1st tranche.

Listed below are names of planholders who are entitled to the said distribution but who have not as yet claimed their checks. Upon claiming their checks, the concerned PPI planholders shall be required to present the following documents: (1) photocopy of plan or contract; (2) photocopy of Certificate of Full Payment (if fully Paid); (3) two (2) valid government-issued id's. If the checks will be claimed by a representative, the representative must present, in addition to the foregoing, a duly authorized Special Power of Attorney and two (2) valid government-issued id's.
