NH Health Care Transition Coalition The NH Health Care Transition Coalition is supported by a partnership of the State Title V program, Special Medical Services and NH Family Voices.

This group meets at least quarterly and is made up of families and professionals interested in Health Care Transition in the state of NH.

The YEAH Council has a great new feature on their Community Page!! They have started an "Ask a Youth" feature where people can post questions related to transition, education, advocacy etc to the council and they will answer it. There is a great thread right now about what parents can be doing to prepare their children for transition from high school. Check it out here:
