A Mitsubishi MU-2B-25 landed gear up at Ji-Paraná, Brazil after the emergency gear handle locked at three pumps instead of the 130 the manual calls for. The shear pin was the wrong alloy. All three aboard walked away. IG-047/CENIPA/2021.

This Cessna 414 gave them a warning on the takeoff roll. They kept accelerating, and then neither pilot could move the yoke. Not the time to find that out.
